Application

To become a member of Wesminster Farm, you must submit these documents with payment:

• Membership Application Form
• Release of Liability
• Membership form for each Dog

Options:

A. Upon completion of forms, mail to Wes Hopkins at W8160 Hwy 11 Delavan, WI 53115 and he will contact you to set an appointment to meet your dog, take payment, and give you a membership dog tags and parking permit.

B. Email completed forms (send as attachments) to Wes Hopkins; You will then receive an email confirmation and an appointment witll be scheduled to meet your dog, take payment, and get your membership dog tags and parking permit.
